We pre-screen all of our recommended services and resources to ensure the safety, quality, dignity and respect your loved one deserves. Every Dementia client’s situation is different and we take pride in personalizing resources to each client’s needs. Your Caregiver Coach will be by your side every step of the way.

 

OUR SERVICES

Weekly Support Groups

Guiding Angels of NH also offers weekly caregiver support groups, online and in-person. The in-person support group takes place every Tuesday from 1:30 pm to 3:00 pm. The online support group takes place every Wednesday from 9:30 am to 11:00 am by Zoom. Contact us for more information. We would love to have you join us and become part of a community that understands your journey.

Case Management

After contacting Guiding Angels of NH, Shirley or Karen will meet with caregivers to do a full intake assessment. It’s important that we get to know your loved one with Dementia so that we can better assess what your needs will be. Once we have a better idea of your needs, we will implement a service/resource care plan to ensure that your needs are being met. Our case management service will monitor your loved one throughout their Dementia journey and continuously evaluate if changes to their care plan are necessary.

When and How to Apply for Disability Social Security

Did you know that when your loved one with Dementia applies for Social Security Disability, they will not receive their first payment until the sixth full month after the disability is approved. Loss of a career or income associated with a Dementia diagnosis can be devastating, especially for those under age 65. At Guiding Angels we have the knowledge and experience to help you with the Social Security process so that your loved one can receive benefits expeditiously.

Estate Planning/Financial Planning

A diagnosis of Dementia can be overwhelming and many of our clients are unprepared to deal with the legal and financial consequences associated with this disease. At Guiding Angels of NH we know the importance of addressing these issues expeditiously and work closely with our clients to make sure they have the needed resources to have plans in place.

 

Legacy Resources

It is often said that legacy work is the gift of love. When your loved one is diagnosed with Dementia, there is no time clock that tells us how long the journey will be. Nothing is more powerful than being able to capture your loved one’s memories before they drift away. Let us help you capture their stories and life lessons so their legacy can live on for generations to come.

 

End of Life Conversations

Although end of life conversations can be difficult, it is important that you know your loved one’s wishes. A Dementia client’s personal, spiritual and cultural beliefs may influence care decisions and multiple conversations may need to occur during a person’s illness. As with any major decision, it is best to have these discussions and make decisions before a health crisis occurs.  Let us help you and your loved one approach these difficult discussions.

Adult Day Services

Adult Day Services provide caregivers the time to work, run errands, attend appointments, socialize with friends, or just take some much needed respite time for themselves. These services also benefit our Dementia clients by providing them with a safe environment to engage in social activities that will help to increase brain stimulation. Some care programs also offer meals and other health related services. Guiding Angels can assist with finding the appropriate Adult Day Service that fits your loved one’s needs.

 

Home care services

Home Care Services benefit Dementia patients by allowing them to remain in their own home. Research has shown that Dementia patients that remain in their own homes have a higher quality of life and live longer. Guiding Angels can help you find the necessary home services to meet your loved one’s home care needs.

 

Meal delivery services

Food Delivery Services can be very beneficial to both Dementia clients and  caregivers. These services ensure that the dementia client is receiving proper nutrition in the comfort of their home. Guiding Angels can connect you with meal delivery services in your area.

Safety Resources

Safety is a priority for Dementia patients, especially as the disease progresses. Guiding angels will come to your home and conduct a full safety evaluation and make recommendations to keep your loved one safe.

Caregiver Resources

Being a caregiver is overwhelming and no two caregivers experience the same journey. We have the experience, both personally and professionally to support you and guide you to the resources necessary to care for yourself. Guiding Angels of NH also offers numerous support groups both in person and online to bring caregivers together in a safe, supportive and private community.

Anticipation of Future Needs

Who will care for your loved one if the primary caregiver has an accident or becomes suddenly ill? Who will care for your loved one when they no longer can be left alone and the primary caregiver has to be away from home (ie:work)? What if my loved one’s condition changes suddenly and I am unable to meet their needs any longer?

 

Guiding Angels can provide the coaching and support needed to anticipate future caregiver needs and help to have a plan in place.
